Personal Data Collection Notice and Consent Form for Personal Data Provision

In accordance with Article 8 of the Personal Data Protection Act, the Taiwan Financial Services Roundtable (hereinafter referred to as "TFSR") hereby informs you of the following matters before collecting your personal data. Please read them carefully.

I. Purpose and Categories of Collection

For the purpose of handling or executing the "FinTechSpace Operation Project," TFSR has commissioned the Institute for Information Industry (hereinafter referred to as "III") and needs to acquire the following categories of your personal data: email address. Your personal data will be processed with the assistance of the commissioned executor, III. If you do not wish to receive further business-related messages from "FinTechSpace" in the future, you may directly click the unsubscribe link provided within the messages upon receipt.

II. Time Period, Territory, Targets, and Methods of Using Personal Data

(1) Time Period: During the "FinTechSpace Phase 4 Operation Project," collected from January 1, 2026, to December 31, 2028.
(2) Territory: Within the sovereign territory of the Republic of China (Taiwan).
(3) Targets: Used for the purpose of handling or executing the "FinTechSpace Operation Project" by III, as commissioned by TFSR.
(4) Methods: Processed and utilized in written, electronic, or other forms.

III. Rights of the Data Subject

You may exercise the following rights by contacting III, the commissioned executor of the TFSR "FinTechSpace Operation Project," in accordance with the rules established for the aforementioned business activities [Contact Window: FinTechSpace, Phone: (02) 2356-9651, Email: fintechspace@iii.org.tw]:

(1) Inquire about or request a review of the data.
(2) Request to make duplications.
(3) Request to supplement or correct the data.
(4) Request to discontinue collection, processing, or utilization of data.

IV. Impact of Failing to Provide Personal Data

If you fail to provide accurate personal data or decline to provide it, TFSR and the commissioned executor, III, will be unable to provide you with the services related to the "FinTechSpace Operation Project" intended by the collection purpose.

V. You understand that this consent form complies with the requirements of the Personal Data Protection Act and related regulations, and you agree that TFSR and the commissioned executor, III, may retain this consent form for future retrieval and verification.

Consent to Provide Personal Data:

1. I have been fully informed and understand the aforementioned notice provided by TFSR and the commissioned executor, III.
2. I consent to the collection, processing, and utilization of my personal data by TFSR and the commissioned executor, III, within the necessary scope of the listed collection purposes.
